Transition Planning

Independent living takes careful planning. We're your transition team, ready to answer questions about adjusting and adapting to new life stages.

  • Caregiver support
    Free education and support for those caring for a senior loved one.

  • Medicare counseling
    Free one-on-one Medicare counseling and education through SHIP.

  • Long-term care advocacy and consultation
    Ombudsmen advocate for the rights of individuals living in nursing home and assisted living facilities, as well as find information about long-term care options.

  • Aging Options Counseling
    Talk one-on-one with our options counselor to discuss resources and solutions for your unique situation.

Volunteer Opportunities

Our volunteers provide countless hours of service to benefit our senior neighbors. Opportunities range from one hour a week to 20 hours a month, or somewhere in between.
