In-home Services

We believe that health happens at home. We are committed to connecting you to non-medical services that help you live more independently at home where you want to be.

If you need additional support to stay in your home, you may be eligible for one of our in-home programs. Call 801-229-3804 to learn more. 

Services that meet your needs

Non-medical, in-home services are available to help seniors remain living independently in their homes. Available services may include:

  • Adult day care
  • Homemaking/chore services
  • Case management
  • Non-medical transportation
  • Incontinence supplies
  • Durable medical equipment

  • Bathing and dressing assistance/personal care
  • Emergency response system
  • Medication management devices
  • Other services as needed

A case manager facilitates services that are offered by a contracted provider. Eligibility requirements apply. 

How to get help

Call our options counselor to discuss your options at 801-229-3804 or email aging@mountainland.org.

After assessing your needs and completing a screening over the phone, you will be placed on the appropriate waiting list.

Once you are accepted onto a program, you will be assigned a case manager who acts as your advocate assisting in developing a care plan to support you through the aging process. Your case manager meets with you regularly to facilitate access to non-medical services to meet your needs.


In-home Programs & Eligibility

We provide case management for the following programs. Please call 801-229-3804 for questions and screening.

Medicaid Waiver

Provides non-medical, in-home services to low-income seniors who are on Medicaid.

  • Must be 65 years of age or older
  • Frailty level that meets skilled nursing facility admission guidelines
  • Medicaid eligible

Alternatives

Provides non-medical, in-home services for older adults at risk of nursing home placement.

  • Must be 18 years of age or older (preference is given to persons 60+)
  • At risk of nursing home placement
  • Meet Federal Poverty Guidelines and have limited assets

Caregiver Support

Offers short-term, in-home services that support caregivers to enable them to care for their loved one and delay facility placement.

  • No income requirement and no fee for participation
  • Caregivers who care for an individual 60+ years of age who is unable to perform routine activities to care for themselves, or caregivers who care for someone of any age with dementia, or caregivers over the age of 55 and caring for an adult with disabilities

Veterans Directed

Gives veterans of all ages the opportunity to receive self-directed home and community-based services that they need.

  • Veterans enrolled in VA Health Care
  • Be at risk of nursing home placement
  • Interested in directing their own care or designating a personal representative.
