STATUS:                         Full Time, Monday-Thursday 7:30 a.m. - 6:00 p.m.

REQUIREMENTS:

Education and Experience:                              

  1. Two (2) years training at an accredited school of nursing (preference may be given to those with a Bachelor’s degree in nursing); and
  2. Two (2) years experience in assessing health needs, experience providing nursing care to elderly population is preferred.

Licensing and Certification:

Must possess a current Utah Registered Nurse (RN) License, AND a current State of Utah Driver’s license.


SELECTION FACTORS:

Skill in current nursing activities including CPR, basic First Aid, blood pressure, etc.  Skill in counseling, evaluation and assessment of client health and program eligibility; coordinate with client physicians to develop appropriate care plans. Ability to be flexible in coordinating work assignments; ability to prioritize work tasks and exercise good judgement.  Strong written and verbal communication skills; excellent organizational skills.  Ability to establish and maintain complete records and files; skill in writing medical reports and case files.  Must be familiar with available resources in the community and serve as an advocate for the elderly.  Ability to maintain cooperative relationships with those contacted in the course of work activities, both internally and externally.  Must be able to operate computer terminal and be familiar with various word processing spreadsheet and database applications, ability to utilize mobile device, must be detailed in completing client files and activity logs.  Must have access to a privately-owned vehicle, and be able to drive to various locations throughout the Utah/Wasatch/Summit county-area as needed.

JOB DESCRIPTION:

Performs a variety of administrative and RN functions related to the support of frail, elderly, program-eligible clients at home, program intended to delay nursing home placement.  Evaluates clients, age 65 and older, to determine eligibility for the Medicaid Aging Waiver Program; determines nursing facility level of care; completes initial comprehensive assessment and periodic reassessments; completes initial comprehensive service plan and periodic updates; and conducts periodic reevaluation of participants’ level of care.  Provides assistance in completing Medicaid application process, consults with case managers, clients, families, physicians, providers, etc. to develop and monitor appropriate care plan that will allow individuals to live as independently as possible in their home setting.  May assist with physical fitness evaluation and health promotion activities at Senior Centers to determine effectiveness and needs of existing programs.

 

PHYSICAL/ENVIRONMENTAL FACTORS: This position requires sitting at a desk or in meetings for extended periods of time, regularly walking and standing, must occasionally lift, carry, push, pull or otherwise move objects weighing up to 20 lbs., regularly drives a motor vehicle within the three county area and potential exposure to illnesses.
